Just because you don't go online on the internet, doesn't mean that your phone doesn't use the internet.

There are a lot of apps out there (more often the free ones) that do use the data connection (i.e. internet usage), plus you would need to check your sync settings as that also uses the data connection, and even if sync is all off, whenever you boot up your phone it does seem to sync date, contacts and e-mail (though it doesn't advise you that you have new e-mail) - take a look at the sync date once you have rebooted - you will notice that it has current date and time, and if you look in battery usage details, there is an option that shows what is using data.

The only way to ensure you don't get charged for data usage is to change the APN - thus ensuring that it can't use any data (careful as this could prevent some aps from working properly).
Menu --> Settings --> Wirelss Controls --> Mobile Networks --> Access Points Names (note the detail in case you do want a data connection - but changing the details so it is incorrect will solve your issue).
